Delaware’s Insurance Commissioner was informed on May 11 that Highmark Blue Cross Blue Shield of Delaware sent explanation of benefits forms to 507 members’ incorrect addresses – the forms did NOT include social security numbers. The company is notifying the affected members by mail – and those went out on May 12. Highmark Delaware discovered the error in March – and the problem was remedied in late April.
